Automatic Renewal: (EFT Only) Provided that Member is not in default of this contract and subject to the terms and conditions hereof, the membership will automatically renew on a month to month basis for the rate indicated below. Membership renewal terms may be cancelled at any time provided a 30-day written notice is delivered to VENT Members Services. Member is required to deliver written notice using the request form on the website www.ventmemberservices.com .
MONTHLY DUES: $120.00 PER MONTH FOR FUTURE AUTOMATIC RENEWAL.
Annual Fee: A yearly fee of $49.00 per contract will be collected. It will be collected 90 days from the date of enrollment and then annually on the same date.

*Up to 5 people per plan. 2 people max, 18 years or older and/or up to 4 people max ages 12-17
